Major Unusual Incidents (MUIs)

A Major Unusual Incident (MUI) is any alleged, suspected, or confirmed event that threatens the health, safety, or well-being of a person with a developmental disability.

MUIs include:

  • Attempted suicide
  • Exploitation
  • Failure to report
  • Missing individual
  • Medical emergency
  • Misappropriation
  • Neglect
  • Peer-to-peer act
  • Physical, sexual, or emotional abuse
  • Prohibited sexual relations
  • Rights Code violations
  • Significant injury
  • Unanticipated hospitalization
  • Unapproved behavioral support
  • Unexplained or unanticipated death

The Major Unusual Incident (MUI) Unit Is Here to Help

If you—or someone with a developmental disability you know—has experienced abuse, neglect, theft, a crime, or any other serious concern, the MUI Unit is here to support you.

Your first priority is safety. Remove yourself and/or the individual with a disability from immediate danger. If necessary, call 9-1-1.

  1. Ensure the health and welfare of the individuals.
  2. Call police if there is a crime or Children Services if the individual is younger than 21.
  3. Report the MUI by calling 419-381-5206 during regular business hours, or 419-380-5100 after hours and holidays.
  4. Notify the guardian, SSA, provider, family, or housemates as soon as possible.

The Ohio Department of Developmental Disabilities also offers a hotline during regular business hours. This option may be used if there are concerns or challenges with reporting to the county board of DD. The Ohio Department of DD hotline number is 1-866-313-6733.
