COVID-19 Vaccine Clinic

The Lucas County Board of Developmental Disabilities, in partnership with the Toledo Lucas County Health Department, is offering a COVID-19 Vaccination Clinic from 9 a.m. to 4 p.m. on Feb. 24. All 3 vaccines, in first doses and/or appropriate boosters, will be available: Pfizer-BioNTech (ages 5+), Moderna (ages 18+) and Johnson & Johnson (ages 18+).

Registration is recommended but will not be taken until Feb. 21 at https://lucascountyhealth.com/covidvaccine/

Walk-ins are also welcome until 3:30 p.m. on the day of the clinic.

For second shots or boosters, a vaccine card is required.

If you've lost your card and were vaccinated in the State of Ohio, the Health Department can look up your record and provide the proper vaccine dosage.

If you've lost your card and were vaccinated outside the State of Ohio, then you will not be able to come get a booster/additional dose.

Photo ID is required. As is a completed Vaccination Registration Form and/or Pfizer-Biontech Covid-19 Vaccine Parental Consent Form for minors getting the vaccine. The forms will also be available on site at the clinic.
