To participate in a sport, you must be at least 8 years old, reside in Lucas County, have a qualifying diagnosis and have a current Special Olympics medical form on file with our office.

Please choose which sport you would like to participate in.

Do you want to participate in Healthy Athletes? (Priority given to those who have not participated before.)

Supervision Requirements

Does the athlete have supervision needs during Special Olympics practices/games (home and away) or while in the community?  

Please note, this form is for registering individuals who are ages 16+. If you are looking for information about the Little Lightning program, please email littlelightning@lucasdd.org.

  • We will do our best to include everyone who signs up by the cutoff date. Please fill out this form online and submit.  This is the quickest delivery.
  • Please, do not call to register. This form must be completed in order to be considered registered.
